Follow the steps that apply to you

I already have a Space NK account and would like to join N.dulge*
Log into your online account here and click on the N.dulge Rewards section in My Account
Click on the Join Now button
Fill out the right-hand side of the form ('Sign up for N.dulge Rewards')
Re-confirm the email address and password for your online account, on the left-hand side of the form
Accept the N.dulge Ts&Cs and click Continue to become a member
